Laparoscopic Groin Hernia Repair Using the Totally Extraperitoneal Approach: A Retrospective Study and Our Experience

Background Recently, laparoscopic totally extraperitoneal (TEP) inguinal hernia repair has been considered one of the most effective and widely performed techniques for repairing hernias by avoiding entry into peritoneal cavity. Its indications have evolved expanded to almost encompass entire range groin hernias. This retrospective study aims determine outcomes postoperative complications in patients undergoing TEP a single surgeon at center. Methodology We retrospectively evaluated prospectively collected data 900 who underwent elective over 18 years center from April 2004 February 2023. Patients were age, sex, type hernia, time taken surgery, open laparoscopy, intra complications, hospital stay, days resume regular activity. Results The mean age was 59 (range = 21-83 years). males females 56 years, respectively. operative 40 55 minutes unilateral bilateral In total, 369 (41%) had right-sided 382 (42%) left-sided 149 (16.5%) A total 121 (13%) occult hernias, 17 concurrent transurethral resection prostate. Of patients, 20 (2.2%) recurrent after previous repair. Seven (0.8%) recurrence post-TEP subsequently (0.7%) needed conversion transabdominal pre-peritoneal approach. Only minor noted postoperatively. average hospitalization 24 hours. normal activities five (±1) days. Conclusions Our experience suggests that with mesh fixation is safe procedure marginal rate. Apart obvious cosmetic benefits minimal tissue invasion, significant advantage visualization contralateral along surgical if required, same sitting without insertion any extra trocars.
